NATIONAL CHAMPIONSHIP APPLICATION

The
fifteenth Futsal (indoor soccer) National Championship games will be held June 23, 24 and 25, 2000, at Alameda Point Gymnasium - 1101 West Red Line Avenue, Alameda Point (7 miles southeast of San Francisco), California. We are now accepting applications for at-large teams that may qualify to participate, on a first come first serve basis. The deadline for notification of intent to participate must be postmarked by June 9, 2000 and sent to the following address:

We anticipate a large response to this National Championship, so be sure to send in your application as soon as possible with a $250.00 non-refundable entry fee for each team.

COMPETITION & AWARDS
.
Each team plays at least three games
Division winners qualify for international play.
Trophies for each first place team.
Medals for all players of the top three teams in each category.



SPECIAL EVENTS

Closing ceremony and awards, Sunday, June 25th.



CHAMPIONSHIP RULES

PLAYER ELIGIBILITY & APPLICATION

.
The roster of each team shall have a maximum of twelve (12) players.
All applications and rosters must be completely filled out at the time of submission or they may be rejected.
All players must be registered with the USFF by the time the application is processed.

PLAYER EJECTION
.
Any player ejected (Red card) from a game shall be barred from participation in the following game.
Two cautions will result in the player being barred from participation in the next game.



SUBSTITUTION:

Unlimited substitution are allowed per game.


TIME OUTS:
.
One time out is allowed per team per half. half.
